One thing you people on the wrong side of the fence keep forgetting is
that the 5' reach is an abstraction. I certainly can't hit someone 5'
away from me with my fist. Likewise the 5' square is an abstraction,
and during a round you can be anywhere within that square, as can an
item that's on the ground. You can be assumed to move over to the
edge of the squares when reaching into another square. Something in
the adjacent square could be anywhere in that other square, so you
could literally have something in the adjacent square at your feet.

The abstraction is irrelevant. It could just as easily be ten feet
away. Just assume it really is 5', and work it out from there.
