The question I always had was -- where in the PH or Dungeon Master's
Guide does it explain exactly who gets multiple attacks per round?

must he be before he can do so? Do the core books explain that

fighters and their subclasses get multiple attacks per round?

PH p. 105, under "First Strike":
The 1 minute melee round assumes much activity -- rushes,
retreats,
feints, parries, checks, and so on. Once during this period each
combatant has the opportunity to get a real blow in. Usually this
is
indicated by initiative, but sometimes other circumstances will
prevail.
High level fighters get multiple blows per round, so they will
usually
strike first and last in a round.

The Fighters' Paladins' & Rangers' Attacks Per Melee Round Table is on
PH p. 25.

This only applies to melee combat. Ranged combat uses the Fire Rate
column on the ranged weapons table on PH p. 38. I'm not sure if this
is stated explicitly, but it is implicit in the table, and it's pretty
darned obvious.

There is a special case in which anyone using a melee weapon might get
multiple attacks: if initiative is tied, and one character has a
weapon with a speed factor at least twice the other's, or 5 greater or
more, the character with the lower speed factor weapon gets two
attacks before the other character's one. If the difference is 10 or
more, the lower speed factor weapon gets two attacks before, and one
attack after. See DMG p. 66 (Weapon Speed Factor).

If you don't care for speed factors, just ignore this section. It was
never intended to be followed religiously; you just use it if you're
interested in the detail.
