That was one thing I liked from one of the NWN modules. There was this
plane, the only way to get there was commit suicide with a certain
dagger, in a certain place, while saying a certain thing.
I've always liked the idea of floating islands in the air. One way to
get from one to another would be to just fall.
In _The Primal Order_ series (from WotC, in fact, before they bought
TSR... and our very own Ryk Spoor helped with parts of them), there
was a book (_Chessboards: PLanes of Possibility) that discussed a
bunch of different worlds/universes/planes/etc. It's worth looking
at, I think. One of the planar ideas sounded something like what
you're describing. IIRC:
There were a 'bottom' and a 'top' of the plane. The top is the sun,
the bottom is... 'mud', or something. It dries out and flakes off,
the pieces rising toward the top. This takes a long time, of course.
The pieces vary in size, but can be large enough to support colonies,
in some cases complete civilizations. Given the time scales involved
(IIRC) it might sometimes be necessary to move (or not, or just the
rich and powerful move, or something -- or everyone just gets cooked
before reaching the top).
