You are taking on a very difficult/confusing subject here.
However, I believe you are confused about some things.
Pathology/laboratory charges: each code/procedure will have
some Medicare allowable charge,and a commercial price. Typically, the
commercial price will be based on a multiplier
of Medicare (eg 145%).
Physician discounting: you are correct, you cannot technically
decrease the price you charge among patients, nor can you
forgive a co-payment. You can, and I do all the time, be selective with the
level of service is coded, which serves
the same purpose.
