Some of the errors I find in patterns can't be determined from a
swatch. I think the one sheephurder mentioned was not a problem of
guage, but that one of the components of a complicated set of separate
pieces that fit together was not the correct size even if the guage
was right.

I find a lot of errors in crochet patterns as well. Sometimes when I'm
almost finished with something I find instructions for the last row or
round that can't be reconciled with what has gone before. I have one
book in particular that is full of errors. Some are egregious, such as
telling you to look at a photo for the placement of something that
doesn't appear in the photo, or telling you to consult a chart for a
stitch pattern, and there is no such chart. Others I have been able to
correct with common sense or devise a work-around. I have the whole
book marked up now with my corrections, and before I start any pattern
in the book, I work through all the instructions mentally, counting
the stitches in each repeat on every row to see if they make sense.
