Couple of possibilities:

1) I'm not too big on iodophor, after early on in my brewing career it
seemed like a few souring bugs escaped the iodophor process. Since
I've switched to Starsan, I've had zilch in infections.

2) Every now and then, I like to soak all equipment that's going to
touch the beer in some PBW (including siphon tubes, stirrers, etc.).
Despite cleaning it by hand, it's my theory that sometimes crud
remains on the various pieces of equipment that can escape your
notice. I may be just anal retentive, but if you have an infection, it
may be worth pulling out your favorite oxygen based cleaner and giving
it a shot.
