It's possible. There's obviously a repeat infection coming from
somewhere. It could be something in your procedure, but more likely
there's a contamination in your equipment somewhere that you're not
getting. It's hard to track this kind of thing down, but I would start
by sanitizing *everything* you use for brewing with something other
than your normal sanitizer. IOW, if you are normally an iodophor guy,
use bleach once and give everything a nice long soak. It's doubtful that
this is the case, but use something different incase you've got a bug
that your regular sanitizer just isn't getting. More likely, there's just
a piece of equipment that you're forgetting, or else some bit inside your
equipment that keeps getting missed. Pay particular attention to the
insides of spigots, etc.
