That's the kind of stuff that paper administrators really go for. In nursing,
we call them "buffed charts". The old saying is, if you didn't write it, you
didn't do it. The unintended corollary is, if I wrote it, prove I *didn't* do
it.

It's all crap. Only the adminstrators seem to be unaware of it.
