Cause of V-22 Unexpected Takeoff and Hard Landing

The latest incident is pretty odd.
Little info is available yet, but it appears that a Marine crew was
testing out an engine on a V-22 and the airplane unexpectedly took
off, rising to 30 feet, and coming down for a hard landing seriously
damaging a wing and engine.
First, how could the A/C take off accidentally?
Second, why would it come down hard?
Could there be a major software glitch that could cause the A/C to
takeoff and interfere with the aircrew attempting to land?
Could the windows have been covered, or some other condition which
would have caused the crew to land hard?
Could it be unfamiliarity of aircrew with the V-22? That could
explain both the accidental takeoff and hard landing (cutting power
suddenly). But how could the airplane get to 30 feet if the aircrew
was in control?
Could the aircrew have started some kind of test without configuring
the airplane properly?
Yes, right now little info is available, this is a speculation post.
It is clear that the actual cause will be available eventually, but
what the heck....
