On rare occasions, I might think the car is in a forward gear instead of
reverse. As soon as I feel the car start moving in the wrong direction,
I take my foot off of the accelerator and hit the brake. I certainly
don't floor the accelerator thinking that it'll stop the car.
Because you're not confused as to which pedal you're using.
But pedal error does happen. Someone gets distracted and puts an
automatic transmission vehicle into gear, it starts to creep, so they
put their foot on the brake -- only it's the accelerator. It starts to
creep more, so they push harder. And so on.
The telling part about unintended acceleration accidents is that they
don't happen in manual transmission vehicles...
