They use a DC-DC converter. It's basically an oscillator that makes AC
which they put that through a transformer to step up the evoltage, then
rectify it to get it back to DC. There's also a tricky voltage doubler
or tripler circuit that doesn't use a transformer that I've seen used
in some consoles.
Another possibility (for the one that uses the two 9V batteries) is
that it doesn't really supply 48V, but rather 18V which will run some,
but not all condenser mics.
