That was a flashback within the series to before the first pilot, and
was new stuff with the new cast. The pilot starts with them already
knowing each other and working at the hospital but not being flatmates
yet.

Are you talking about the original pilot that someone just mentioned,
which was then recast? Because from the very first episode that aired
in the U.S., all three of them were living together. If you saw a
pirated version of the UK pilot, then that's a different discussion.
AFAIK, folks here were talking about the U.S. airings.
