Don't ask. The show was completely in disagreement on it. So were the 2 women that wrote the novels. One insisted that it was happening the way you said, and Sam was really there, and somehow the clothes fit, and the other said nothing but Sam's mind moved, and seeing him was just for the benefit of us and Al. The second woman was proven wrong a couple times, like in the Jennifer Aniston green mini skirt episode, where Sam was in an amputee and ran around and everybody thought he was levitating.

I always figured it was just Sam "fitting" into the other body, like in the Oswald episode and they had the "actual" Oswald in a holding room. But then, where was Sam's body when he was jumping around into other bodies. Very confusing!
