It's an assumption on your part that if a child uses a first name vs. a
title, "the respect isn't there" -- that may or may not be true.

*Children* don't need to call an adult by something other than their
first name in order to understand that they are not equals ... children
don't even THINK about things like "being equal to an adult" unless an
adult *teaches* them that.

Children should be and some *are* taught to respect adults that are
worthy of respect regardless of what their names or titles are. Some
adults prefer to be called by their first names and others prefer to be
called Auntie ... if the child has been taught to respect adults,
shouldn't make a difference whether he calls her Sue or Auntie Sue.

I'm not saying it's a bad thing for kids to be taught to call their
grandparents "Grandma" and "Grandpa" ... just that saying there's no
respect there if they grew up calling them something else (like their
first names) is not always the case.
