Actually, I don't have much of an issue with the 2 years life span.
I am concerned about brand new lamps that failed as soon as I
started to refocus the fixture. The 2 suggestions that make the
most sense so far were that the single bar that holds up the
filament can be a problem if it winds up below the filament,
although you report that in your experience single large filament
lamps are just as rugged as the folded filament lamps. The
other is to check the centering of the lamp in the reflector
because the envelope may be bumping the edge of the opening.

I went back through the thread and you never said who manufactured the
fixture, or what it was. I don't want to assume they're source 4's.
GLA's are hardy and I've never specified whether the filament support
should be below or above because the axial units can rotate 360
degrees, so it's a non-issue.
Also, you said you haven't measured voltage at the fixture. You need
to do that. Did you recently have any work done on the dimmer rack?
Were new circuits added or dimmers repaired/replaced?
I've had nightmares created by people who get busy hands when I'm not
around and I come in to find disasters. One theatre hired someone
other than me to specify and install all new dimmers. When it was
done, they had been shortchanged on the number of circuits and
dimmers, circuit numbers did not jibe, and all the plugs for the
overhead strip lights were melted. Yes, I said melted. I didn't take
any pictures because that was before cell phone cameras, but I will
next time.
Check the basics, I'm incredibly skeptical that it is the lamps fault
(unless you just got a bad batch made by the new guy on the line that
day).
