I disagree. You have a single document that contains the notes.
You
pass that around with the document that's being reviewed. When
done,
pass the document back to the author. He makes changes from the
notes, makes comments in the notes, and passes it back out for
another round. Nothing gets lost or missed, and something as
simple
as putting your initials next to each comment ensures that
attribution is preserved. It's as easy as having them inline, and
avoids the dysfunctionality of software like Word because it can
be
done with plain text or whatever other format you like.

Ah, OK. I thought you meant "everyone with comments e-mails them".
Still, think how convenient it is that newsreaders allow us to
place
comments inline naturally, rather than having to refer to "line 12,
para 1 of Mike's third post of the current subthread."

You are completely right. I apologize humbly and profusely for have
had the temerity to doubt your wisdom. Please
forgive me.

Also, having a defined format for comments vs. text, as well as
keeping a change history, preserves the integrity of the original
document. Some people, after all, might be tempted to slip in edits
for their own purposes.
