Baloney. It can ,mean exactly that. Order at most gives you a hint of the
penumbra of connotation, but cannot be relied on.
I'm not sure if it's still the case, but in the UK, advertising rules
meant that the item featured first in a pair should be dominant. So,
for example, a chicken and ham pie should have more chicken than ham.
Applying those rules to the original example [1], a science fiction
and fantasy author would write more science fiction than fantasy.
OTOH a fantasy and science fiction author would write more fantasy
than science fiction.
