Let's not forget the hundreds of millions the drug companies spend on
developing new drugs that are marginally, if any, better than existing
ones... on which the patent is about to run out.

With a few exceptions (Loratadine/Desloratadine comes to mind) I suspect
that the drug companies don't intend to come up with marginally improved
treatments. OTOH, once they've spent hundreds millions of dollars
developing a new drug they're likely to try and go to market if there's
any improvment at all. If nothing else they may find a market in people
that don't react well to other treatments. Example: Loratadine does
absolutely nothing for my Dad, fexofenadine works well.
