Ummm, lessee - atmospheric pressure is about ten meters - 33 feet of

I checked it again, and you're right, I slipped a decimal point.
So if I'm having trouble talking with .1% of an atmosphere pressure
difference, whatever difference Heinlein might have been thinking of
would pretty certainly be out of the question.
Leaving aside the fact that once you approach 10% of an atmosphere
pressure difference, your lungs have ruptured and you are experiencing
a surprisingly painful death by drowning in blood.
I can't think of a scenario in which an ambiently pressurized oxygen
mask isn't good enough, but an oxygen mask that adds pressure in your
lungs -- within safe limits -- is. The safe limits are too low.
