But you can get form mania, especially when you have bureaucrats in
different offices all collecting slightly different bits of information
about the activities of the same unfortunate target group.

Oh yeah it drives me nuts when I go to the doctor's office and they hand
me four different forms, all asking for the same information, but with the
order of the blanks randomized from form to form. One has first name last,
the next first name first, the third has it both ways, in various places
on all fourteen pages. All forms of course designed in Word by secretaries
with a glancing acquaintance with spelling and none at all with logical
information organization.

When you ask them, they just say "oh we need the information in different
places for various purposes" at which I blow my top and ask if any of them
have ever HEARD of a database? Like you could input it once and output it
different ways? Huh?

I love the blank looks. I suspect they have extra forms for people like me
who complain. "Here, we need this too. For the circular file. Press hard,
it has 6 copies."

The medical profession is the last to computerize their stuff. My
opthalmologist is just learning her system which was installed only a couple
months ago. Paper charting is still going on at lots of places like the
nursing home my mother lived in. I asked why they did it that way and they
couldn;t say. Take care
