Like your scenario above, keeping government out doesn't mean there
is nobody else affecting things. It just means that the someone
affecting people's lives is a corporation trying to make a buck off it.

The only way forhealthcare to be efficient and cheap is for there to
be one really huge provider, a monopoly of sorts, then the economy of
scale would take care of things. Otherwise, it's just companies trying
to figure out how to make the most profit hwile providing the least
service.

But the only way to make a monopoly play honestly is to take profit
entirely out of the equasion, either by making it a government-run one
or by regfulating the hell out of it.
