Oh I doubt /that/ completely. I imagine that RTD thought that it would be cool to bring back the Master, then figure out what he thinks is the most diabolical situation imaginable, then write from the middle /outwards/.

Ends are usually easiest to wrote, both front ends and back ends, it's the bit in the middle where they join that can be quite sticky.

On the basis of LotTL I'd agree with your first statement. He knocked the ending out in a lunch break, it seems. Making things join up is where RTD falls flat on his face - once again, we agree. He writes the equivalent of a big bit of sellotape to hold the two disjointed edges of the story together then sticks a puppet on his hand and gets us all to look at the funny monkey, hoping we might not notice. :-)
