Thanks! I guess the important thing is that the statement is signed by a notary. I hadn't thought of that.

Actually, you aren't *required* to have anything, but in the event of an emergency, you may find having the child's health insurance information and a notarized "minor authorization" letter. The letter is pretty much this:

As for air travel, there is no need for ID or any sort of proof of age or even permission to travel for children under age 18.
