Why assume Lay didn't know he had advanced coronary artery disease? He
may have been in denial, or may have been afraid that it would be a
sign of weakness. Or, like a lot of people, rich or otherwise, he may
simply have assumed that ignorance is bliss.

He apparently had at least one heart attack before the Big One. Most
heart attacks are hard to ignore, and evidence of any heart attack
would show up on subsequent EKG's.

Jim Fixx the "fitness through running" guru dropped dead from a massive
coronary while out running. The autopsy revealed he'd had more than
one serious heart attack. He apparently kept them secret in order to
maintain his image as a fitness god.

Any competent doctor can tell a lot about your heart's condition from a
12 lead EKG.
