This is not remotely settled. There are all sorts of variations of the
Arthurian myth, and quite a few of them - indeed, probably more of them
than not - do not have Excalibur having been created by Merlin; for one
thing, I'd doubt that pretty much any of the ones where it's handed over
by the Lady of the Lake use that origin.

Well, the movie Excalibur, which was sort of a Greatest Hits
compilation of the legends, had Merlin imbed it in the stone to be
freed by Arthur, then, after Arthur accidentally broke it, the Lady of
the Lake repaired it and handed it over, so it neatly threw in both
stories (yeah, yeah, "I saw it in a movie, so it must be true"...).
Interestingly, the movie did not actually say that Merlin did create
the sword, or where it came from, IIRC...

Then again, there's the Dr. Who story where "Merlin" is an alias a
future Doctor uses, and he did create the sword, which was a
technological marvel...

A lot of the variations of the Excalibur story are that it's the sword of King Arthur's adulthood, with the sword in the stone being the sword of his youth.
