Evangelion Death and Rebirth question



I watched the Manga DVD of DEATH AND REBIRTH last night and was
reminded that it's two pieces combined, a 69 minute compilation of
selected moments from the series, followed by its own end credits and
then an intermission and then an approximately 26-minute preview of
THE END OF EVANGELION (basically the first 26 minutes of END OF...).

So here's my question: Was DEATH AND REBIRTH released in Japan the
same way it is on the DVD or were the two pieces first released in
theaters separately from each other?
