White thread like worms in the tank



How do I get rid of them? I think I got them last time I add some
plants in my tank (few months ago). They don't show up all the time
but some times you can find one or two of them swiming in the water.
Very short and thin, like a white thread. My fish sometimes eat it if
they happen to see it. I read somewhere that they are harmless. I
just want to get rid of them.

I have a 125 g tank, changing 30 to 40% water twice a week, ph 7.8
(tried very hard to get it dwon and had to give up), no ammo, temp 88
F. 6 discus (about 3" each) in the tank and some plants. I used to set
the temp at 84 to 85 F then I read somewhere that most nasty thing in
the water can be killed at 86F or higher and my discus like the 88 F,
they look much happer.

I feed them 3 times a day, BW in the morning and afternoon, flakes 90
minutes before lights off.
