The ones I've seen it's pretty much anywhere in the cycle. I've been
sitting at red lights and had it suddenly change to blinking red or
yellow.
Most of ones I've seen usually change during the 2 second "all red"
interval.
On the thru roads it's usually : Green-yellow-red (pause)-yellow flash
On the side roads it's: steady red- red flash
End of flash: yellow flash-red-green (thru); red flash-steady red (side)
This is what I've seen; in fact, anything else is MUTCD non-compliant
(see my comments elsewhere in this thread).
Another aside: One alternative to flashing mode that I read about on
this group (can't find the post after multiple searches, but I think
it was suburban Sacramento) is red for every direction until a vehicle
approaches. This way no one has to wait for cross traffic to get a
yellow then red.
