Stimulus Payment For Teenager?

I have a son who is now 13 years old. For tax year and prior, I filed a form 1040A for him as he had investment income. That changed during 2004 and there has been no need to file a tax return for him ever since as he no longer has investment income.
Today I received a blank Form 1040A in an evenlope marked "STIMULUS PAYMENT" with a notice to file this and mark STIMULUS PAYMENT on it. HUH???????????
First of all, does my kid qualify for the bucks? Our (wife and I)taxable income is over $200,000. Certainly WE don't qualify. Second of all, if he DOES qualify, do I just send a form 1040A in with his name and address and SS number and no other data?
A little more info on this is appreciated.
