1. How much it will cost to replace my passport
Depends on your home country. You should contact the consulate of your
home country
2. If I need to leave the country to go to canada/Uk etc to get the visa
stamp in my passport
If you leave the country, then you will need a US visa to come back.
However, you do not need a US visa to stay in the US.
The US consulate in your home country is the only one which has the
obligation to process (note that I said process and not approve) your
visa application. Other US consulates may process it if they're nice
with you - but there's no guarantee of that.
3. How long the process takes
That would be a regular visa application. That greatly depends on your
citizenship.
4. I have no other form of id here, is ths going to be a problem???
In most cases, you can use your passport as a form of ID while within
the US.
You can use the approval notice to show that you are legally present in
the US.
It would be good if you have a copy of your I-94 ; you can surrender
that when leaving the US in order to have your departure recorded (and
avoid being accused of overstays)
