Mistake in H1B Extension app. Please Help!

My employer just filed for my 8th year extension. But I there was a
mistake in the application. Here is the background.
I have made two trips outside the US in the last 2 years. I went to
Sydney Australia in October 2004 and returned with a new H1B visa and
was given a new I-94 card. Then this June 2006 I went to Toronto Canada.
When I made that trio my H1B was expired. According to the rule for
traveling to Canada and returning within 30 days, I did not surrender
my I-94 at the airport. I stayed in Toronto for 3 days and returned to
the US with my expired H1B and Valid I-94. The US immigration checks
were done in the Toronto airport. They checked my passport, my expired
H1B and my valid I-94 and allowed me to board the flight. I was not
given a new I-94.
Now the issue is due to miscommunication my employer put October 2004 as
the last date I landed in the US in my H1B extension application. We
realized this after we mailed the application. They told me not to worry
about this. They said that USCIS will send a RFE and then we can explain
the mistake.
My questions are :
1) Can my extension be denied due to this mistake?
2) Since I was not given a new I-94 when I returned from Toronto, what
proof can I show for that last landing?
Anything you can tell me about this situation is appreciated!
