effect but at the same time use it in their own system

It is very simple. Homeopaths do not consider their remedies to be
placebos; they are depending on it to work. When an MD intentionally
gives a placebo, he is doing so with exactly that expectation. It is
very telling that this simple fact didn't occur to you.
How very curious.
