credit card question - confused about master card #

I've always been taught and experienced that the first digit of a cc
is call the "system #".
visa - 4
mastercard -5
amex - 3
discover -6011

Got a phone order today. Lady had a mastercard that started with a 4.
Refused to belive it. She faxed me a copy of the card and sure enought
it looked like a mastercard but started with a 4.

Any ideas????
