What app to edit an interactive PDF?

What application is necessary to edit an interactive PDF (if I have
the correct name)?

I have a PDF at http://www.geol.ucsb.edu/administrative/field_reservation.pdf

that needs to be edited. For anyone who may be an expert at this,
could you perhaps download it and tell me what application is needed
to modify this file? I just need to make the "Submit by Email" button
actually work again as well as modify one of the pop-down menus.
