I had a similar event with my MBP. System volume controls showed maximum
level but sound was not as loud as usual. Opened iTunes to listen to
some music and noticed the volume here was set at about 50%. Bumped up
the slider. It makes no sense to me why or how but it worked and I have
full volume back.

Background:

-I use Airport Express and remote speakers so had "allow iTunes control
from remote speakers" enabled. I set this preference a long time ago
(before upgrading to Leopard and before many Itunes/QT updates). Am
guessing something snarfed volume levels during that string of upgrades
and updates.

-On my system, the controls for system and iTunes volume levels were
always in sync. This was the first time I noticed them being out of
sync. I was out of the habit of visually checking the two and SOP was
using the keyboard to adjust volume. It was a fluke that I even glanced
at the iTunes volume controls but am glad that I did.

-Since manually "re-connecting" the sound control levels in iTunes and
system, they are once again in sync.

Sorry this is not an exact answer or even a technically sound one but
since I also experienced a loss in volume, I've taken the time to tell
you how it sorted out on my system.
