did you move safari from its original installed location in the
applications folder to some place else? if so, move it back.

I'd put my browsers in a Browser folder in my Applications folder. That
approach helps me three ways:

1. I'm not confronted with a long list of applications.

2. If I forget the name of an application, I can still find it.

3. If I forget about a useful application, I'll spot it the next time I
got to the folder of similar applications.

Except an occasional installation problem, is it bad to group my
applications in subfolders?

While nice for you, it causes problems when you attempt to update
software and the files it is looking for in standard locations can
not be found.

I have seen systems totally screwed up by moving stuff out of the
Applications and Utilities folders.

Better approach. Create a folder(s). Put aliases of the apps
you want in the folder(s). Put the folder(s) somewhere you find
convenient. Leave the original files/applications where their
installers put them.

Creating an alias for an applications is as simple as Holding
Option-Command while dragging the application to the destination
folder. Instead of moving the application, it will create an
alias with the same name as the application.
