For some reason, my Apple menu reveals duplicate entries. And whenever I

You pressed the Command and F5 keys on your keyboard.
Command-F5 turns on "VoiceOver," which is designed to allow visually
handicapped users to use their Macs by voice.
You can turn it off by clicking on System Preferences and using the
Accessibility pane. You can turn off Command-F5 by clicking on System
Preferences, choosing the Keyboard and Mouse preference pane, selecting
Keyboard Shortcuts, and disabling the option to switch VoiceOver with
Command-F5.
