I keep a "minimal" dock and use LaunchBar for launching applications. That
way my dock should contain only icons for the currently active applications.
However, at least since Tiger came out and perhaps even before, the icon for
Safari will remain in the dock when it is closed. Removing the Safari icon
from the dock will remove it only until the next time Safari is used. This
behavior is unique to Safari and doesn't occur with any other of the
applications I run.

As far as launching applications when their icon is not in the dock, and if
you don't use such as LaunchBar for launching, I recommend placing the
Applications folder in the dock. Clicking on that folder icon then produces a
list of all of the installed applications, and clicking on one of the
applications listed will launch it.
