What caused an outlining rectangle & double entries under the apple?



A couple of days ago I played with the system preference, Universal
Access to magnify a picture on a site. I also ran ClamXav. Since near
that day I have double entries for Restart, shutdown, & log out under
the blue apple, and a rectangle that outlines what it wants to in the
finder and most apps. I have searched these 2 apps to see what could
have caused the differences (double entries & black rectangle), but can
find nothing. I have restarted several times, and repaired
permissions. Nothing makes a difference. It is also possible that it
happened before that, and I just didn't notice it.

I would appreciate any clues you can give me.

I am running OS X 10.4.6 on a dual 2 GHz PowerPC.

    ... Why do you want to perform a restart when apps are *running*? ... Because I assume it is like Windows, in that when you issue a restart each App is *requested to exit*. ... it is just apps that can be quit with no prompts. ...
