I use Spamprobe (with Procmail). It seems to do the job.
My setup uses Procmail to sort messages into mbox files
to be read using Mutt.

I haven't compared Spamprobe to other programs, so there
may be alternatives that are more effective and easier
to use.

It's a Bayesian filter, so the user doesn't have to make
up rules to distinguish spam from nonspam. Just tell
Spamprobe about false negatives and false positives and
it will adapt.
