Question about keyboard shortcuts

I made a couple of keyboard shortcuts to menu commands but they don't
always work. They're both for specific applications and both were made
in the Keyboard & Mouse pane of System Prefs.
One is in Safari. It activates the Clear History menu item. About half
the time it works, otherwise I just get an alert sound. When it doesn't
work, I open and close the History menu and then the shortcut works.
BTW the keyboard stroke I chose in option-command-H, which I'm pretty
sure is not used by any preexisting menu item.
The other shortcut is for MS Word. It's supposed to activate the Save
As... menu item. The keyboard stroke I chose is command-shift-S. But
usually it selects the Style field in a tool bar, and I can't find a
preexisting shortcut for that (although admittedly I'm pretty
Word-ignorant). Similarly to the Safari example, when it doesn't work I
open and close the File menu and then the shortcut works.
