The lips appear in the top pain of the mail, to the right of the address
fields.

That's where Mail.app shows any image/photo associated with the sender
of each email, so presumably she's somehow associated the 'lips' with
her own profile in her address book. Solution is to open the address
book and delete the image, I guess.

The image is not attached to the email in any way, so the recipients
will never see it.
