Any MPEG-2 file that you can "watch" with audio tends to be
multiplexed. When not multiplexed, the separate streams are referred to
as "elementary streams." MPEG-2 video streams typically have the
extension m2v -- if you see the extension mpg, the file is almost
certainly muxed.
VOB files are just the Video OBject files that comprise content on a
DVD...they can be either MPEG-2 or MPEG-1 video with PCM, AC3, or mp2
audio. NTSC conformance says that at least one audio track must be PCM
or AC3 -- PAL doesn't have that restriction and, in fact, most players
will play NTSC DVDs with mp2 audio even if no AC3 or PCM track is
present.
MPEG-2 is also used for SVCD and HD transport streams (different frame
sizes and bit-rates, of course).
