Hello
I have a laptop which I have been using for 2 years. Over the past 2
months, the machine has started to show that the ac adaptor is
connected to the machine, even when the power is disconnected. The
problem is that as the machine considers itself to be charging all the
time, it doesn't give any alerts when the battery is about to die, and
I keep losing my work.
I have tried rebuilding the machine and changing the battery. Neither
helped.
Any ideas?

No, rebuilding the machine and charging the battery won't help. As either the battery is shot, and/or the charging system is defective. Personally, I think you have two problems going there. As a bad battery always charging can take out the charging system too.
Thus why you should never use a weak battery and never use the laptop on AC while the battery isn't being charged. Some argues that if the power fails or the power plug gets accidentally unplugged, your laptop turns off without a battery.
Well this is true. For the first one, I like some others use a UPS. I also plug in my cordless phone, lamp and alarm clock into it as well. Both the cordless phone and the alarm clock will run days without power on the UPS. I also use my laptop on a desktop (most of the time) with external monitor, keyboard, TV tuner, and mouse. Disconnecting a few cables and it is truly portable. And it is impossible in either case to have the plug accidentally unplugged.
