Any ideas as where to start looking will be greatly appreciated.
I already tried to change the SCSI cables, and the controller.

I had this problem a few times while installing NIKE arrays and moving the system volume set out to them. Each time the problem turned out to be the firmware on the SCSI card I was using to attach the array to the MPE box. Check the firmware on your SCSI card.
