Dial-up Modem loses Connection

Hi
I have a Magic Express dial-up modem connected to the serial port of my
StrongArm RiscPC with RISC OS Adjust 4.39 and using Voyager a my Internet
connection.
Just recently, over a year or so, the OH LED on the modem goes when I'm
online. I'm assuming that OH stands for On Hook meaning there's a
connection between my computer and the telephone system when the LED is lit
and no connection when the LED is unlit.
My question is fairly simple, though the answer may not be:
Why does the modem keep losing losing the connection?
I am aware that it can happen anywhere in the chain between me and the
remote site I'm trying to access.
