The page size code is device dependent, so it would be an adaptation

But there may be extra considerations e.g. in Windows you can't freely
add sizes. You may have to define forms.

The best driver is the right one. Reading back this is for distilling,
so the Distiller driver is the right one. It supports custom sizes if
the driver/application does.
(other than the one that might correspond to the RIP that will be
used.
Yes, if it is going to the RIP directly, that is the only driver to
use. No excuses, RIPs are not generic. But if distilling, it's
Distiller that is the immediate RIP.
