Your image 'black' is 63% C; 51% M; 51% Y; 91% K. Total ink coverage of 257%
(or so :)
It is a rich black - sort of. It's an image.
Your top rectangle is 100% black in CMYK output.
You're comparing apples to oranges.
Look at your separation preview under the advanced menu and you will see
that it is 100% black. Don't worry about the colour on your screen.
