Absolutely not. There is no reason in DICOM of using subdirectories,
and if yes you can use whatever structure you want.
Anyway, it is wise to put the DICOM files (except the DICOMDIR, that
must be in the root) in subdirectories that are below a given
directory in the root, for practical reasons. First of all, some
removable media format allows only a limited number of files in the
root (for example, FAT16, used for the MO disks, allows only 512
files); then, IHE (PDI integration profile, radiology domain) says:
"The DICOM instance files shall not be in the root directory or in the
IHE_PDI sub-directory, instead they shall reside in a sub-directory
whose name is not otherwise constrained."
Giving that, you can decide to create the subdirectory structure that
you prefer, not exceding of course the constraints about the name and
the maximum depth that you find in the DICOM standard.
