With regards to a) - check out Udo's goodies
(http://readthesourceluke.blogspot.com/) there is an ResourceChanging
Image stripper - if you use that it can automatically bind your icons
into the resulting .exe (search for posts on this on the group - from
about 2 years ago).

Its a very cool addition to Dolphin (thanks Udo) - in fact the Cincom
guys were talking about how they package apps (from James' podcast a few
weeks ago) for windows and it was so convoluted compared to this
(launching external tools and hacking exe headers etc).
